Contract Rules  

 

Application

  • All forms must be completed and returned to Mel Family Daycare before a child enrolls in our child care services.
  • All forms are subject to yearly renewal and must be kept up to date. This includes health, parent pick-up authorization, and transport forms.
  • All families who wish to enroll their child in Mel Family Daycare will begin with a two week trial, in order to ensure proper adjustment. During this two week trial, either the parent(s) or I may terminate application and enrollment, at any time.
  • All families who complete their two week trial will be enrolled into Mel Family Daycare.  If a family wishes to terminate enrollment at Mel Family Daycare, a two week notice is required.  If Mel Family Daycare wishes to terminate enrollment at Mel Family Daycare, a two week notice will be given.

Hours & Fees

  • Our regular hours are Monday through Friday, from 7:00 a.m. to 5:45 p.m. 
  • Regular Full Time enrollees are those who require childcare from 7:00 a.m. to 4:30 p.m.
  • Extended Full Time enrollees are those who require childcare beyond 4:30 p.m.
  • Part Time enrollees are those who require three or less days per week.  Part Time enrollees who wish to change days must provide two days notice, and receive approval from Mel Family Daycare.
  • After School Enrollees are those who require childcare from 2:00 p.m. to 5:45 p.m.
  • Late Pick-Up Fees and Extended Time Fees apply to ALL enrollees who are NOT picked up by your scheduled time.
  • You will be charged an additional $10.00 for the first 15 minutes (5:45 p.m. to 6:00 p.m.) and an additional $1.00 for every minute after 6:00 p.m.

HOLIDAYS & VACATIONS

  • Mel Family Daycare is closed for the following holidays:
    • New Years Day – January 1st
    • Memorial Day - May
    • Independence Day – July 4th
    • Labor Day - September
    • Veteran’s Day - November
    • Thanksgiving Weekend - November, Thursday and Friday.
    • Christmas Eve - December 24th we will close at 2:00 p.m.
    • Christmas Day - December 25th
    • New Year Eve - December 31st we close at 2:00 p.m.
  • You will be billed for these days.
  • If you’re your child is absent due to vacationing or any other personal reasons, please notify us as soon as possible.
  • You will be billed for these days. 
  • If I decide to take an additional vacation day, or need to deal with any personal matters during the week, I will find a qualified substitute who will come to my home and care for your child. I will give you two days notice, when possible.

ILLNESS, SICK DAYS & MEDICATION

  • Parents must notify Mel Family Daycare if your child has been ill at home.
  • If you are notified by Mel Family Daycare that your child is ill, he/she must be picked up as soon as possible.
  • Notification is given if your child has a high fever, if his/her illness can endanger other children, or if he/she requires special medical attention.
  • You will be billed for your child’s sick days.
  • Medication will be administered by Mel Family Daycare ONLY with a Medical Release Form and written permission from parents.
  • For additional information, please read the attached “Emergency Plan and Health Policy”.
  • IF I AM SICK, I will find a qualified substitute who will come to my home and care for your child. I will give you two days notice, when possible.

MEALS

  • Mel Family Daycare will provide up to three meals a day:
    Breakfast, lunch, and an afternoon snack.
  • Menus for the following week will be available each Monday.
  • Mel Family Daycare does NOT provide formula, or infant cereal.   
  • Parents with infants and children who require formula or infant cereal must supply their own formula or infant cereal which we will prepare for your child.
  • At your request we can provide “home food” for your infant.  Ask for details.
  • If there are special food requests, parents must provide the required food items. 

SUPPLIES, WHAT TO BRING

  • Each child at Mel Family Daycare will be assigned a cubby for his/her supplies.  Please be sure to label ALL of your child’s items with his / her name.
  • REQUIRED ITEMS:
    • One (1) change of clothes.
    • One (1) bottle or cup.
  • REQUIRED ITEMS IF NOT TOILET TRAINNED:
    • Diapers / Wet Wipes / Diaper Ointment (optional).
    • Parents will be notified when additional supplies are required.
    • Parents who do not provide additional supplies when required will charged for extra supplies provided by Mel Family Daycare.
  • OPTIONAL ITEMS:
    • Blanket or pacifiers.
  • DO NOT BRING:
    • Books or toys, we have plenty for your child at Mel Family Daycare
    • Expensive jewelry, value items or electronics.
    • We assume NO responsibility for such items, nor will we be held responsible for any damage or loss.

DISCIPLINE

  • Mel Family Daycare will discipline your child in any manner deemed necessary which is NOT abusive and will NOT involve corporal punishment.
  • If you have any special concerns about discipline or your child’s behavior, we are available to discuss those concerns with you.

DROP OFF, PICK UP & TRAFFIC RULES

  • When dropping off or picking up your child, please follow these guidelines.
    • Be courteous to our neighbors.
    • Park on Palms Blvd. or on the east side of Rosewood.
    • Park on Palms Blvd. if you plan to stay longer than 5 minutes.
  • If you park on Rosewood Ave., in front of Mel Family Daycare: 
    • DO NOT park longer than 5 minutes when dropping off or picking up your child.
    • DO NOT park more than 2 cars.
    • DO NOT park too close to the corner of Palms & Rosewood.
    • NO U-turns, NO double Parking, NO blocking ANY driveways.
  • As a personal request, DO NOT park in front of 3493 Rosewood Avenue.  This is a special request from our neighbor and we wish to avoid conflicts.
  • If you do not follow these rules, Mel Family Daycare will terminate this contract, and we will be unable to provide care for your child.

VISITATION, TRANSPORTATION & CHILD RELEASE

  • Parents are welcome to visit their child, at any time and without notice, during our regularly scheduled hours.
  • If you require Mel Family Daycare to provide transportation for your child in the case of an emergency, you must sign and complete a “Consent to Transport” form.
    • This form will be supplied by Mel Family Daycare, and is attached for reference.
  • Mel Family Daycare will NOT release your child to ANYONE who is NOT on the “Identification and Emergency Information Form.”
    • You MUST notify us in writing if you wish to release your child to another adult. 
    • You MUST notify us in writing if there are individuals who are NOT permitted to pick up your child.

MISCELLANEOUS

  1. Any modification or changes to this contract MUST be made in writing and signed by both Mel Family Daycare and the parents of the enrolled children.


  2.  I have read and understand the terms and conditions of this contract with Mel Family Daycare.  I agree with the terms and condition so f this contract with Mel Family Daycare.
